Evidence summary of nutritional management of food for special medical purpose in postoperative adjuvant chemotherapy patients with cancer

Objective To summarize the best evidence of nutritional support therapy with food for special medical purpose for patients undergoing adjuvant chemotherapy after oncology surgery,and to provide a reference basis for standardizing the application of food for special medical purpose and guiding clinical nutritional therapy practice for oncology patients. Methods Computer search was conducted on UpToDate,Guidelines International Network(GIN),National Guideline Clearinghouse,National Comprehensive Cancer Network,National Institute for Health and Clinical Excellence,Oncology Nursing Society,American Society for Parenteral Enteral Nutrition,European Society for Clinical Nutrition and Metabolism,Chinese Society for Parenteral and Enteral Nutrition,Joanna Briggs Institute Evidence-Based Health Care Centre database,Cochrane Library,PubMed,CINAHL,Scopus,Web of Science,Embase,CBM,Wanfang,and China Knowledge Network on the nutritional support treatment of food for special medical purpose for postoperative adjuvant chemotherapy patients. The retrieval period was from January 2012 to August 2022. Quality evaluation of the literature was completed independently by 2 evidence-based trained researchers and combined with expert advice to extract and summarise evidence for the literature that met quality standards. Results A total of 14 articles were included,including 4 guidelines,1 clinical decision,2 systematic reviews,4 expert consensuses,and 3 randomized controlled trials. The 14 pieces of best evidence were compiled,covering 6 aspects of nutritional screening and assessment,nutritional education,recommended dose,formulation selection,nutritional treatment,and management of adverse effects. Conclusion This study summarises the best evidence on the nutritional management of food for special medical purpose in postoperative adjuvant chemotherapy patients with cancer,which is scientific and systematic. When administering nutritional therapy to patients,healthcare professionals should fully assess the nutritional status of patients,make clear and prudent choices on the dosage and type of food for special medical purpose to be used by patients,and guide patients to improve their nutritional status safely and effectively to improve their quality of life. To improve the quality of life.

Nursing care of a case of intimal hyperplasia at buttonhole puncture site of arteriovenous fistula

The nursing experience of intimal hyperplasia at buttonhole puncture site in a patient with autogenous arteriovenous fistula was reported. The key points of nursing:to formulate a scientific and reasonable internal fistula puncture plan,to establish and maintain the buttonhole tunnel,to regularly monitor the use of arteriovenous fistula,to replace the traditional internal fistula steel needle(hereinafter referred to as the steel needle) with the hemodialysis trocar needle (hereinafter referred to as the trocar needle) for buttonhole puncture,to treat with far infrared ray during each dialysis,and to guide the patient to apply hirudoid cream on the arm of the fistula side. After careful nursing,the intimal hyperplasia at the buttonhole puncture site disappeared,and there was no recurrence after 6 months of follow-up.

Construction and evaluation of a risk prediction model for hypoglycemia in colonoscopy patients

Objective To analyze the influencing factors of hypoglycemia in patients undergoing colonoscopy and to construct a risk prediction model and evaluate the model. Methods A total of 528 patients who underwent colonoscopy were selected by the convenience sampling method from the gastroenterology department of a tertiary A hospital in Qingdao from March 2022 to August 2022. Their general information,laboratory indicators and operation-related data were collected. Multivariate Logistic regression was used to analyze the risk factors of hypoglycemia in patients with colonoscopy for risk prediction model construction,and its prediction effect was evaluated by drawing a nomogram. Results Hypoglycemia occurred in 66 of 528 patients,with an incidence of 12.50%. The risk factors finally in the risk prediction model in Logistic regression were drinking history,long fasting time after operation,polyethylene glycol(PEG)-electrolyte solutions>3 L,low quality of bowel preparation. The model passed Hosmer-Lemeshow goodness of fit test χ2=10.158(P=0.200). The area under the ROC curve was 0.829,while the cut-off was 0.575,with sensitivity of 92.90% and specificity of 64.60%. Conclusion Patients undergoing colonoscopy have a higher risk of hypoglycemia. Patients with a history of drinking,longer fasting after surgery,more than 3 L of PEG-electrolyte solutions,and low quality of bowel preparation were more likely to develop hypoglycemia. The established risk prediction model has a good effect,providing the reference for screening high-risk group of hypoglycemia and taking preventive and protective measures.

Safety planning intervention can provide patients with personalized coping strategies during suicidal ideation and suicide-related behaviors,thus preventing patients from committing suicide. This paper summarizes the concept,development,implementation content,advantages and application effect of safety plan intervention,in order to provide references for constructing and perfecting the suicide prevention training program for patients in China.

Dealing with dysphagia plays an important role in oral feeding of infants after congenital heart disease(CHD) surgery. Identifying related risks to dysphagia and taking early intervention measure can reduce the risk of malnutrition caused by dysphagia to improve the overall prognosis. As there are few domestic related studies,this paper is mainly based on foreign studies,in order to study postoperative dysphagia in infants with CHD from the perspective of the main symptoms,current situation,risk factors,assessment tools and inspection items,intervention strategies,and continuity management. Finally,the contribution of this article mainly focuses on proposing suggestions on how to improve clinical practice and manage infants with dysphagia after CHD operations in China.

Construction of sexual health care training program for cervical cancer patients

Objective To construct a training program of sexual health care for patients with cervical cancer for gynecological oncology nurses,and provide references for the theoretical research and clinical practice of improving the sexual health care ability of gynecological oncology nurses for patients with cervical cancer. Methods Based on literature review and qualitative interview,the first draft of a letter inquiring about the training program of sexual health care on cervical cancer patients for gynecological oncology nurses was formulated. From September to December 2020,15 gynecological oncology medical experts,gynecological oncology nursing experts and sexual science experts from 4 provinces and cities of Beijing,Tianjin,Shanghai and Jilin Province were interviewed by Delphi experts for 2 rounds to formulate the training program of sexual health care on cervical cancer patients for gynecological oncology nurses. Results The effective recovery rates of 2 rounds of expert consultations were 93.75% and 100%. The authority coefficients of the authority coefficients were 0.843 and 0.853.The Kendall’ W was 0.429 and 0.326. The coefficient of variation of items in the second round was 0~0.115,and the full score rate is 20%~100%.The constructed training program of sexual health care on cervical cancer patients for gynecological oncology nurses consists of 3 parts,among which the training content includes 6 first-level indicators,16 second-level indicators,48 third-level indicators,and 6 training methods and 6 assessment methods. Conclusion The process of constructing training program of sexual health care on cervical cancer patients for gynecological oncology nurses is authentic and reliable,with distinctive specialty features,various training methods and assessment methods,which can meet the needs of sexual health care training for gynecological oncology nurses,and can promote the scientific and standardized development of sexual health care.

Objectives To explore the level of resilience among adolescents with Type 1 diabetes and its association with glycemic control,diabetes distress,and quality of life. Methods A total of 360 adolescents with Type 1 diabetes were recruited from the outpatient and inpatient in Endocrinology departments of 2 public hospitals in Nanjing by convenience sampling method. The data were collected by the Diabetes Strengths and Resilience Measure for Adolescents,the Problem Areas in Diabetes Scale-5,the Short Form of the Chinese version Diabetes Quality of Life for Youth Scale,and the Self-Designed General Information Questionnaire. Results The resilience of adolescents with Type 1 diabetes was 40.18±9.27,which was above the medium level. Resilience was negatively correlated with glycosylated hemoglobin value(r=-0.373,P<0.001) and diabetes distress(r=-0.336,P<0.001),while it was positively correlated with quality of life(r=0.431,P<0.001). Hierarchical regression analysis showed that resilience independently explained 10.0% of the variation in glycemic control,7.8% of diabetes distress,and 17.7% of the quality of life,after controlling for demographic and disease variables. Conclusions Interventions that target patients’ resilience may improve their glycemic control,mental health,and quality of life,which should arouse the attention of medical workers.

Posttraumatic growth experience of grandparents of children with developmental disabilities under 6 years old:a qualitative study

Objective To explore the posttraumatic growth experience of grandparents of children with developmental disabilities under 6 years old,so as to provide a basis for formulating intervention programs to promote their psychological growth. Methods The purposive sampling method was adopted to select 12 grandparents of children with developmental disabilities who received the rehabilitation treatment in a tertiary A hospital in Chongqing from October 2021 to January 2022 as research subjects. Data were collected by semi-structured interviews and face-to-face nonverbal behavior observation method,and data were sorted out with the help of GooSeeker software. The data were analyzed by Colaizzi 7-step analysis method. Results 5 themes were extracted.①The positive psychological derivations:perceived progress of children,looking forward to a better future,positive coping styles.②New life perception:accepting reality calmly,shifting focus of life,valuing health more. ③The enhancement of personal strength:the enhancement of resilience,the enhancement of self-efficacy,optimism and prospect. ④The enhancement of support system:the enhancement of emotional support,the enhancement of social support.⑤Interpersonal harmony:altruistic behavior,improvements in relationships with others. Conclusion Grandparents of children with developmental disabilities under 6 years old have various experiences of posttraumatic growth. Medical staff can help them get more social support through group psychological counseling,setting up psychological support groups and strengthening disease publicity,so as to help them effectively cope with stress and promote posttraumatic growth.

Evidence summary for prevention and management of feeding intolerance in preterm infants

Objective To select and obtain the best evidence on management and prevention of feeding intolerance in preterm infants,and to summarize the best evidence available. Methods We searched the UpToData,Joanna Briggs Institute(JBI),Guidelines International Network(GIN),National Guideline Clearing house(NGC),National Institute for Health and Care Excellence(NICE),Registered Nurses’ Association of Ontario(RNAO),Scottish Intercollegiate Guidelines Network(SIGN),Cochrane Library,PubMed,Embase,CINAHL,American Society for Nutrition(ASN),and complementally searched the CNKI,the CBM and WanFang database,to collect relevant guideline,evidence summary,best practice information sheet,recommended practice,systematic review,expert consensus and original study. The time limit for the retrieval is from the inception of databases until August 30,2019. 2 investigators evaluated the quality of the included literature and extracted evidence from documents that met the quality criteria. Results Totally 11 articles were selected,and 18 pieces of best evidence were summarized,including the influencing factors of feeding intolerance,evaluation criteria,prevention,management and outcome indicators. Conclusion Our research summed up the best evidence on prevention and management of feeding intolerance in premature infants. In order to improve the quality of nursing care,selective evidence should be applied with the consideration of environment,conditions and patient’s willingness.

Objective To analyze the risks in a temporarily renovated ICU at Tongji Hospital in Wuhan using the Hazard Vulnerability Analysis based on the Kaiser Model,and to propose strategies to respond and manage the risks. Methods The risk management index evaluation system for nursing management was developed based on the Kaiser Model and the status of the temporarily renovated ICU at Tongji Hospital. It was surveyed via web questionnaires during Feb 21st to 25th,2020 among the medical teams in the ICU,and the index risk value and matrix distribution were analyzed. Results The top 13 risk events perceived by the medical team were listed as following.C8 nurses needle stick injury(0.447 6),B1 patients need to be rescued (0.413 0),C10 nurses being infected (0.411 0),B2 patients’ psychological problems(0.407 4),C9 doctors being infected(0.359 8),E3 insufficient or interrupted oxygen supply(0.329 1),D9 drug shortage(0.324 2),E2 insufficient protective equipment(0.317 9);C12 doctors and nurses’ absence from work due to emergency illness(0.309 1),C7 doctors needle stick injury(0.298 2),E1 insufficient supporting staff in the hospital(0.293 5),B6 adverse drug reactions in patients(0.292 4),C6 nurses’ psychological maladjustment(0.289 1). Conclusion Managers should pay attention to the risk assessment on the status of the temporarily renovated ICU at Tongji Hospital,and then guide the problem solving; at the same time,they should attach great importance to the occupational protection and physical and mental health of medical staff.

The effects of oropharyngeal colostrum administration in preterm infants:a systematic review

Objective To systematically evaluate the effects of oral care using colostrum in preterm infants.Methods We searched databases including PubMed,EMBASE,Cochrane Library,Web of Science,CINAHL,CBM,CNKI and Wanfang. We included all randomized controlled trials and clinical controlled trials which using colostrum in preterm infants’ oral care. The quality of articles was appraised by the Cochrane Handbook(5.1.0),and RevMan 5.3 software was used to analyze the data.Results A total of 10 studies were included. The results showed that oral care using colostrum reduced the occurrence rate of sepsis[RR=0.54,95%CI(0.36,0.79),P=0.002]and feeding intolerance[RR=0.59,95%CI(0.40,0.87),P=0.008],shorten the time to reach full enteral feeding[MD=-3.70,95%CI(-6.38,-1.02),P=0.007]. At the same time,the detection rate of pathogenic microorganisms and bacteria in oropharynx decreased,and the level of sIgA in saliva and urine increased. There was no significant difference in occurrence rate of necrotizing enterocolitis[RR=0.78,95%CI(0.43,1.42),P=0.420],time to start enteral nutrition [MD=0.07,95%CI(-0.67,0.81),P=0.850],and time to start oral feeding [MD=-2.45,95%CI(-4.99,0.09),P=0.060].Conclusion Oral care using colostrum can reduce feeding related complications in preterm infants,accelerate feeding process to a certain degree,and enhance immune levels of preemies. More high-quality,large sample studies are needed in order to further explore the effects and the most effective way of oral care using colostrum in preterm infants.
